FLORIDA DRIVER LICENSE AND IDENTIFICATION CARD GENERAL INFORMATION
NOTE: Effective September 1, 2009, most motor vehicle and driver license fees increased significantly. These fee increases were passed by the Florida Legislature and approved by the Governor to help balance the state budget. None of the additional money will stay with Pasco County Government or the Tax Collector's Office. FAQS for Driver License Changes Effective October 1, 2008
If your Florida License is currently suspended, revoked or cancelled; or you
have received correspondence from the Florida Division of Driver Licenses; or to
obtain information regarding your driver license history:
    You may request this information in person at our office. Personal Identification is required.  OR
    You may request this information by telephone from the Florida Division of Driver Licenses.
    Contact Customer Service at (850) 617-2000, between 8:00 a.m. and 5:00 p.m., Monday thru Friday.
If your Florida License or Identification Card has been lost or stolen:
    If your Florida Driver License or Identification Card has been lost or stolen, you may
    obtain a replacement at our office by providing the following:
        • United States Birth Certificate (original or certified copy),  OR
        • Valid United States Passport   OR
        • Social Security Card, or any of the following documents showing your social security number:
           tax return, W-2 form, property tax statement, pay check stub, DD-214, or school record   OR
        • Motor vehicle registration with Florida Driver License Number printed on it,  OR
        • Payroll stub not over 30 days old with Social Security Number printed on it,  OR
        • Life Insurance Policy reflecting Date of Birth. Policy must have been in effect a
           minimum of two years,  OR
        • You may obtain a replacement by mail from Florida Division of Driver Licenses. Contact
           Customer Service at (850) 617-2000, between 8:00 a.m and 5:00 p.m., Monday thru
           Friday.
    If an address change is requested, proof of residential address is required. Provide any of the following reflecting
    your name and address: deed, mortgage statement,lease agreement, voter registration, vehicle registration, W2 form,

To change your name or address on a Florida License or Identification Card:

    Address Change Information:
         A replacement Florida Driver License or Identification Card with a new address and photo may be
         obtained at our office.
         Proof of residential address is required. Provide any of the following reflecting your name and address: deed,
         mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form or a
         homeowners, automobile, or life insurance policy. For a complete list of items, click

         You may complete and mail a Division of Driver License Mail-In Address Change Form to Tallahassee
         with the required for a Driver License or an Identification Card. Either a new license or
         a decal reflecting your new address will be provided,  OR
         You may change your address over the Internet at www.flhsmv.gov.

    Name Change Information:
         A name change requires the issuance of a new Driver License or Identification Card.
         A replacement with new name and photo may be obtained at our office.

         and address: deed, mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form or a

         An original or certified copy of your Marriage License or Court Order reflecting your new name
         must be submitted. Photocopies cannot be accepted.
         A change of name cannot be processed by mail or Internet.

To Renew your Florida Driver License:
    Driver License renewal forms are mailed directly from the Florida Division of Driver Licenses in Tallahassee .
    The renewal form will indicate whether you may renew by mail, or if you must renew in person.
    To Renew by Mail/Internet:
        State Policy does not permit Tax Collectors to renew Driver Licenses by mail. You may mail your renewal
        card and check to the Division's Tallahassee address provided on the card,  OR
        You may renew over the Internet at the web address provided on the renewal card, or
        at www.flhsmv.gov.
        Further information regarding renewal by mail may be obtained from the Florida Division of Driver Licenses.

    To Renew In Person:
         If required, or if you choose, you may renew in person at our office. Florida law requires an additional
         $6.25 service charge for renewal at any Tax Collector Office. If citizenship status has changed since
         original issuance, documentation is required.
         Proof of residential address is required. Provide any of the following reflecting your name and address: deed,
         mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form or a
         homeowners, automobile, or life insurance policy. For a complete list of items,click Proof of Residential Address.
         Social Security Card, or any of the following documents showing your social security number:
         tax return, W-2 form, property tax statement, pay check stub, DD-214, or school record.
         Our following offices provide Driver License services:
            • New Port Richey, Tax Collector Building, 4720 U.S. Highway 19, Gulf Harbors.
            • Dade City, East Pasco Government Center, Room 100, 14236 6th Street.
            • Land O' Lakes, Central Pasco Government Center, 4111 Land O'Lakes Boulevard (U.S. Highway 41).
         Driver License written or vehicle test must begin no later than one hour before office closing time. No appointment is necessary.
To Renew your Florida Identification Card:
    The Florida Division of Driver Licenses does not mail renewal notices for Identification Cards.
    Identification Cards may be renewed in person at our office. The expiration date will be
    found on the card.
