MOTOR VEHICLE DUPLICATE TITLE |
| To apply for a duplicate title, complete Form 82101 and sign where indicated. The form may be downloaded from the Division of Motor Vehicles' website at http://www.flhsmv.gov/forms/duptitle.html. If the address listed on the application is different from Department of Motor Vehicle records, proof of address change is required. See form for a list of acceptable documents. |

| The title should be issued by the Florida Division of Highway Safety and Motor Vehicles in Tallahassee within 3 weeks. |

| Fast title service is also available and the title should be issued by the Pasco County Tax Collector's Office within 5 working days. To expedite your Fast Title application, please make a notation on the outside of the envelope, Attn: Fast Title Service. |

| NOTE: Effective September 1, 2009, most motor vehicle and driver license fees increased significantly. These fee increases were passed by the Florida Legislature and approved by the Governor to help balance the state budget. None of the additional money will stay with Pasco County Government or the Tax Collector's Office.
